When doctors are wrong.

I recently watched this video–a TED talk–by Dr. Peter Attia. You may have seen it as it’s become quite a viral sensation over the last few weeks. But even if you have seen it, you might find it useful to watch again. So here it is:

While I don’t agree with absolutely everything Dr. Attia has to say, I do think he brings up a few important points.  One issue is that some doctors, scientists, and other medical professionals are really starting to question the causal nature of the link between obesity and diabetes.  I think this is an important area that will require a lot more study.  And I think it is our job to continue to push for this continued study.

But one issue that I want to particularly want to highlight here is how hard it seems to be for doctors to admit they are wrong.  Dr. Attia is clearly deeply moved.  He feels a tremendous sense of remorse for how he treated that poor woman with diabetes.  Once he realized the level to which he had allowed stigma to affect his treatment of this woman he was devastated.

Many of us would be quick to state, well he should be.  He may have deeply hurt this woman.  He may not have given her the best medical care.  Many of us don’t go to the doctor because we are so afraid of being hurt just this way at the doctor’s office or the hospital.  Some of us have died because of this.

To which I would respond, “Yes, that’s true.”

But I think it’s also important to see what this video has to teach us about doctors and what it might be like for them to understand that they were wrong about something.  We look to doctors to fix everything.  We ask them to make us well and to bring us back from the brink of death.  It takes a certain amount of arrogance to hold a person’s beating heart in your hand and endeavor to fix it.  And I imagine there is a certain amount of pain when you have to tell somebody or tell their family that you can’t fix it.  You can’t make it all better.  You are not god.  And I’m not sure that the pain ever goes away.

Please understand.  I am not making excuses for doctors who bully and stigmatize fat people.  It is wrong, and it needs to stop.  Now.  That is why I am working so closely with the Size Diversity Task Force and the Association for Size Diversity And Health on the Resolved project.  We need to share our stories.  We need doctors and the public to understand that weight stigma is extremely damaging to fat people in medical settings and is sometimes even fatal.  There was a period of years in my life when I was quite sick and might have died based on the assumptions that doctors had made about me.  So I get it.  This must change.

But I think, if we want our work to be effective, if we want things to change, we need to be perceptive and understand what it means to help doctors understand that they are wrong about this.  We need to understand this–not so we can let them off the hook–not so we can let them down easy– so we can find the best path towards an actual solution, so we can understand why many doctors are so resistant, and so we can better understand why this is taking so long.

The issue of weight stigma in medicine is complex and nuanced.  But I do know one thing.  It will only change if a lot of us continue to work together to bring about change.  I would love to hear your thoughts about this issue.  And I would love to have your continued support to make the Resolved project a success!  Click here for more information about how you can participate.

On Doggie Diversity

frenchiemirror copyIt’s always amazing to me, just how hard it is for people to wrap their heads around size diversity.  They readily accept that some people are tall and some people are short.  They accept that some people are good at math and some people can sing.  They understand that we come with different eye colors and different hair colors and textures and have different shoe sizes.  But when it comes to clothing sizes, when it comes to body composition–there is one size to rule them all.  And that size is small.

Why, oh why is it so hard to accept that there will be diversity in body weight and shape as well as height and color?  Some dogs are greyhounds.

pitbullGreyhounds are not better than pit bulls.  They aren’t healthier.  In fact, pit bills on average live longer than greyhounds do.

No matter how much a pit bull diets, it will not be a greyhound.  It will just be a very, very thin pit bull.  The very thin pit bull will not be morally superior to other pit bulls.  It will not be healthier than other pit bulls.  In fact, it will probably be pretty sickly compared to other pit bulls and unless it is fed enough food, it will die of malnutrition and starvation.

And frankly, the notion that extremely svelte pit bulls look better in designer clothes is just a myth:

The reality is that the world is a place of wondrous diversity.  We come in all sizes, shapes, colors and types.  And that diversity is awesome.  It makes life interesting.  It helps us survive as a species.

doggydiversitySo instead of disparaging people who look a little different, let’s celebrate our diversity!  Then we can all run and frolic and play nice together.

Recently, I hosted a Size Diversity Task Force meeting and a Canadian documentary film crew at our house.  They were interviewing us regarding our Paper Mache in a Big, Big Way project and also about size stigma in general.  One of the questions that came up really struck me.  The question was, “Do you also feel pressure to fulfill what some might consider to be a “positive” association with being fat.  For example, do you feel pressure to be jolly?  And if so, is this a bad thing?”

I’ve given a lot of thought to this question over the years.  And at least for me, the answers in order are, “Yes, yes and often times.”  I would say that over the years, I have felt pressure to present myself as a “good fatty”.  This means presenting myself as somebody who exercises and eats well.  This means presenting myself as somebody who is happy and well dressed and tastefully accessorized, and frankly, not too much trouble to anyone.

And yes, I’ve felt pressured to present myself as jolly.  In the past this meant carefully avoiding the expression of anger and the assiduous avoidance of the “angry fat woman” stereotype.  It also meant feeling the need to be entertaining.  If you look at the few fat people on TV and on the stage, if  you look for the role that the world asks us to play, you will see many of us in the role of comic relief.  It seems that we are allowed a small toe-hold, a small part to play as long as we are funny–as long as we are entertaining.

So for many years, I found myself playing the role of the funny fatty.  It was as if I was apologizing for not being pretty to look at and compensating by at least being fun to be around.  I learned to tell a good story.  I learned self-deprecating humor.  I learned to make people laugh.

And in many ways, this is a good thing.  It is a good thing to be entertaining.  It is good to laugh.  It is good to be able to make people laugh.

But it also became a way that I experienced oppression.  While it is wonderful to make people laugh, it is not so wonderful to feel like you must make people laugh.  I often found myself calculating my worth based on whether or not I was entertaining.  I felt like being funny was my job, my justification for being, the shield that would protect me, and the platform from which it was okay for my fat body to be in the world.

It was exhausting.

And this is why the expectation of jolliness is sometimes bad.  Sure it’s great to have the option to be jolly.  But once there is an obligation to be jolly there is a problem.  Because nobody feels jolly all the time.  And everybody should be allowed to express a full range of emotions including happy and sad and angry and tired and everything else.  Every BODY has a right to exist whether they are funny or not.  And I think sometimes, this pressure to be jolly is about not wanting to address the stigma and the pain fat people face in our society.  Because it’s difficult to express just how angry you are about being called names, or not getting good health care or not being able to find nice clothes or not getting a good job when you are under a societal mandate to be jolly.

So, my dear friends, if you feel jolly, by all means, be jolly.  If  you feel sad, feel sad.  If you’re mad as hell and don’t want to take it any more, then stand up sister, rise up brother, and be heard!  Because big, little, fat, skinny, tall or small, your feelings are important.  You are important.  So please feel free, to simply be…you.

Paging Doctor Good…

Sometimes fat chicks have to shop a little harder to find a good doctor…

 

Well, yet another study has come out indicating that fat stigma is alive and well among medical students.  Even NPR (with it’s RWJ-fueled, anti-fat slant) released a story about this study conducted among medical students in South Carolina.  Over one third of the students tested, demonstrated moderate to strong bias against obese people.  What’s more two thirds of those students who demonstrated fat bias were even aware that they had any bias against overweight and obese subjects.  And while the study only checked bias among students in one school, it is clearly in line with other studies that demonstrate fat bias among doctors and med students.

None of this comes as much of a surprise to overweight and obese people seeking health care in this country.  Many of us have experienced not only a flat refusal to treat the illness or injury for which we are seeking care, but also outright and overt disdain from our doctors.  I have spoken extensively about my own story in the past.  And as I have reviewed the videos that have been submitted for the Resolved Project created by the Size Diversity Task Force and ASDAH, I’ve cataloged so many distressing stories.  We are compiling a veritable library of cases of little kids put on speed, doctors refusing care and medically supervised fat shaming.

It’s so hard to find a good doctor that will treat us with respect.  It can be a long and arduous search.  So it’s probably not a shock that another report was released last week that shows that overweight and obese patients are more likely to engage in “doctor shopping”.  If one in three doctors coming out of medical school are displaying recognized or unrecognized bias against people of size, it makes sense that we have to look a little harder.  What I find interesting about the study however is the way it connects doctor shopping with quality of care.  One of the lead doctors from the Johns Hopkins study, Dr. Kimberly A. Gudzune, stated:

“There’s something going wrong in these doctor-patient relationships that make these switches so frequent for this group of people… The real problem here is that the health of overweight and obese patients who doctor shop is being compromised. Because they do not remain with their doctors for very long, they are ending up in the emergency room, likely for things that could have been taken care of in a primary care office.”

I wonder, does this point to yet another reason why fat stigma may be causing health problems that are currently blamed on fat?  I’ve stated before, and I’ll state again, there is no proof that weight loss improves health.  (For one thing, we can’t get enough people to keep enough weight off for long enough to do a real test.)  We also know that people who engage in healthy behavior live longer, regardless of body size.  What would be very interesting to me would be to see what impact medical training to reduce weight stigma would have on the health of fat patients.  That’s the study I would like to see.

In the meantime, I guess we’ll just have to keep searching for Doctor Good…

Talking about Bullying

This past Saturday, I  had the great privilege to speak on behalf of the Size Diversity Task Force at the Stop The Pain Anti Bullying Conference in Riverside.  I spoke about all bodies being good bodies and I talked with them about the fact that weight-related bullying can come from many sources including parents, teachers, doctors and coaches.  It seemed like many of them were interested in the topic and I had a great time.  But one of the moments that really touched me, came from a Dad in the back of the room.

The room was overfilled and he had stood against the wall for the entire presentation.  At the end I asked for questions and he raised his hand.  He said that he had come to the presentation on behalf of his sixteen year old daughter.  He said that he came with his beautiful wife (and he gestured towards her) because he wanted to know how to help his daughter.  He said that his daughter had always been somewhat heavier, and that she was having a hard time accepting herself.  He said that he and his wife told her that she was beautiful every day, but that she was having a hard time believing it.  He wanted to know what he should do.

I told him first of all, that he should keep telling her that she is beautiful.  That maybe it doesn’t seem like it’s sinking in, but that she’s hearing it.  I told her that sometimes we aren’t ready for that message at that moment in our lives, but that there will come a moment at some time in our lives when we are ready, and we will gather those words and those memories to us at that time and we will treasure them always.  I told him that he could offer help, but that he has to be patient.  She will only accept help when she’s ready.  I suggested that he could direct her towards support groups like the Size Diversity Task Force that could some day help her find her way through the prejudice out there and help her emerge triumphant as a size positive freedom fighter.  And I thanked him for asking the question.

Honestly, I was blown away by this guy and his wife.  They cared enough about their daughter to stand in a hot classroom for an hour and listen to me talk about my journey, size diversity, size prejudice, the near impossibility of permanent weight loss, Health At Every Size and more.  They were able to keep their minds open and see if they could learn something new.  And they were humble enough to ask for help.  Now I have no idea what it is really like for them in their house and in their family.  But I was deeply moved by the idea that there are parents out there willing to buck the status quo to really help their kids and there is hope in this world of size oppression within strong families willing to care for their kids in a way that may not be “socially acceptable” but in a way that works.

I am deeply grateful to Kandee Lewis and the Size Diversity Task Force for this opportunity to speak, but more importantly to listen and learn from other folks who are working to end bullying in their own lives and the lives of others.

Stuff that Weighs More Than Me: The Biggest Pot of Tea in Boston

If you’re having a craving for tea, I’ve got just the thing!  If you go to the corner of Court and Tremont Streets in Boston and look up, you’re likely to see this huge teapot steaming away.  Built by Hicks & Badger in 1873, this giant kettle has been a Boston tourist attraction for many years. The teapot has an apparatus inside that makes it “steam” year round.  Locals say the steam is much more pronounced in the colder months.

The “Big Teapot” is perhaps best known for its part in a major publicity stunt held in 1875, when a contest was held to guess the exact capacity of the giant kettle.  Over 13,000 guesses were submitted and more than 10,000 spectators filled the square on the day the capacity of the kettle was measured.  It took over an hour for Boston’s Sealer of Weights and Measures to fill the kettle and learn exactly how much it would hold.  At 1:05 the total was announced: 227 gallons, 2 quarts, 1 pint and 3 gills.

We don’t know the actual weight of the teapot itself, so I’m just going to calculate the weight of the tea that would fit in the pot:

227 gallons=1890.91 pounds

2 quarts= 4.165 pounds

1 pint=1.04375 pounds (at room temperature and sea level)

3 gills (each of which is about 5 oz.)= 0.978 pounds (at room temperature and sea level)

For a grand total of 1897.09675 lbs.

Conclusion: This pot of tea weighs more than me.

Teenage Boy “Diagnosed Fat”–Infection Missed

chart2How many times have we heard this same story?  A vibrant, active young man goes to the doctor, in this case, with knee pain.  The doctor does a routine test and doesn’t see a problem.  The doctor does see a young man who fits into an “undesirable” segment of the BMI chart.  The solution, the young man is diagnosed as fat, is told that his “extra weight” is probably causing the pain in his knee and sent home.  Now Kaleb is an active kid.  He plays rugby, and he loves to sail.  But once the doctor sees Kaleb’s place on the BMI scale, he just might think something like this: “Aha!  I don’t have to say that I don’t know what’s wrong with this kid’s knee.  I can write obesity in the kid’s chart and then we have a diagnosis!”

Unfortunately, in Kaleb’s case there was another diagnosis besides “fat” to be found.  A short while later, he was taken to the hospital via ambulance after he fell down some steps.  At that point, he was referred to a specialist that he saw two weeks later.  The specialist ordered an MRI and during the scan they found a serious bone infection.  Kaleb was scheduled for emergency surgery the same night.  He is recovering well.  So thankfully, the story has a happy ending.

But how much pain could have been avoided without the “fat diagnosis”? It appears that had this infection been detected earlier, it could have been treated with antibiotics rather than emergency surgery.  Now there’s no guarantee that had Kaleb been thin, they would have found the infection sooner.  They might have still sent him home and told him to take some aspirin and take it easy.  Thin people are misdiagnosed too.  But I’ve heard time and time again about people who are “diagnosed fat” and sent home.  Remember this guy who was diagnosed fat, and it turned out to be a brain tumor?  Remember his emergency surgery?  I wonder if doctors, frustrated by a lack of diagnosis and discouraged from ordering expensive tests don’t lean on the BMI chart as a way to have something to write in their diagnosis box.  I imagine in many cases, once patients are “diagnosed fat” and are shamed and blamed, they stop asking annoying questions.  They stop demanding that doctors figure out what is wrong with them.  In some cases, they stop going to the doctor altogether.  This is part of the collateral damage and opportunity costs in the “war on obesity”.  This is another example of the casualties that arise from singling out a body type as unacceptable and trying to eradicate it.

And we’re not just dealing with misdiagnosis here.  We’re dealing with fat people suffering and dying from the mutilation of otherwise healthy tissue via gastric bypass and banding surgeries.  We are seeing the development of more and more new strategies for trying to make fat people “healthy” by making their digestive systems mimic eating disorders and limited blood flow to the gut.  We are so focused on helping fat people get healthy by making them thin that we are willing to make them really, really sick to help them get there.  And sadly, in so many cases, the fat people who undergo these treatments end up fatter or sicker or less happy than they were in the first place.

There are weapons we can use in this war.  One of them is to ask the doctor if thin people also experience the same problem.  In Kaleb’s case, he or his mom might have asked, “Do thin people also have knee pain?  What tests might you do if I were thin.  Can we do those tests please?”

Another weapon is to help make doctors and other medical professionals more aware of the pain and repercussions of fat bias.  And it just so happens that we have some terrific tools to do that.  The Association for Size Diversity And Health (ASDAH) along with the Size Diversity Task Force are compiling videos about fat bias in healthcare.  The project is called RESOLVED.  Some folks at ASDAH have informed me that the deadlines are being extended.  You can hear more about the project and see my sample video HERE.  In addition, the Size Diversity Task Force has a unique opportunity through one of our members to help train medical advocates about fat bias in healthcare.  But in order for your video to be used in both places, you need to submit your video by March 18.  If you’re interested in participating in the project and/or have any questions or concerns, please leave me a note in the comments below.  Or send me an email at jeanette at thefatchick dot com.  I’d be glad to help.

Let’s do what we can to limit the number of casualties in the war on fat.  Let’s help kids like Kaleb get the attention and care they need at the first doctor’s appointment–not the third.  Let’s see what we can do to have “diagnosed as fat” be a thing of the past!

A Fatty Affair 2013 was Off the Hook!

If, like me, you were privileged enough to go to A Fatty Affair last weekend, I imagine you are still basking in the afterglow of what can only be called a massively cool rad fatty event.  If you didn’t get to go, I’m sorry.  But you can still view the clip above to get an idea of just how much awesome and win was experienced by all.

There was singing and dancing.  There was talk about sex.  There was home-baked yummy desserts.  There were hula hoops.  There were butt bounces.  There was laughing and playing and more fabulous clothes for swapping than you can even imagine.

Kudos go out to Sarah Redman and her entire planning committee for creating such an uplifting, warm and positive event.  I can’t wait for next year!  Thankfully, Sarah has informed  us that they plan on doing even more events in the coming year.  If you want to stay on top of all the stuff this great group is cooking up, check out their web site.

This weekend reminded me once again, just how powerful community can be.  I think at one time or another all of us feel like we are all alone in our journey towards self acceptance.  But there’s no reason to go it alone.  In addition to the Fatty Affair folks, there’s the Size Diversity Task Force (who has already collected over 15,000 pages towards the Paper Mache in a Big, Big Way project), ASDAH, The Fat Chick Clique and The Fit Fatties Forum (who have reached the Rockies in their jaunt across the USA).  Don’t worry if one particular group is too far away or doesn’t fit your style.  There are amazing size acceptance groups everywhere!  So don’t try to do it all by yourself.  Share or be square!

The Right Now! Show: On the Size Diversity Task Force


This week on episode 2 of “The Right Now Show with Jeanette DePatie (AKA The Fat Chick) I’ll be talking about a fabulous activism project created by the Size Diversity Task Force (of which I am a proud member).  The Size Diversity Task Force is asking folks to “liberate” diet books from thrift shops and rummage sales and donate the pages to the SDTF.  The group will then take the pages and use them to create a world-record breaking paper-mache sculpture!  You can learn more about it here:
